Vital Statistics

Marriage applications will be scheduled by appointment only.

This office is responsible for the issuance of certified birth, death, marriage certificates and domestic partnership certificates.

 The following is a fillable form which you can complete, print and mail with your payment. (You may also call the Vital Statistics office to request copies.)

The fees for certified copies are as follows:

Birth Certificate - $10.00 each.
Marriage Certificate - $10.00 each.
Death Certificate - $10.00 each., corrections $5.00
Domestic Partnership - $10.00 each.

Please provide a stamped/self-addressed envelope.

 

Marriage-Civil Union License Requirements

Applications are taken by appointment only. Couples may make an appointment for the application up to six months in advance but cannot pick up the license any earlier than 30 days prior to the wedding. The State requires a 72 hour waiting period from date of application to date of issuance.

Couples must bring with them the following:

  • Driver’s License as proof of residency or a Lease or a Bill with your address
  • Birth Certificates
  • Social Security Numbers
  • Witness over the age of 18 (can be a family member)
  • Names of both sets of parents and the State or Country where they were born
  • Maiden names of both Mothers
  • Name and address of person performing the ceremony and where wedding/union is taking place. (Township)
  • Where there was a previous marriage or civil union, please bring divorce papers or death certificate
  • The fee is $28.00. Cash or Check is acceptable

The application can be taken in the following places:

  • The Township where the Bride resides, or 
  • If the Bride/Applicant B is from out-of-state, the Township where the Groom/Applicant A resides, or
  • If both Bride/Applicant B and Groom/Applicant A are from out-of-state, in the Township where the marriage/union is to be performed.
  • In this case, the license is valid only in that Township.

If the Bride/Applicant B or Groom/Applicant A is under the age of 18 but over the age of 16, parental consent by both parents is required. Along with the parents, two witnesses to the signing of the parental consent form are needed at the time of the application.

If the Bride/Applicant B or Groom/Applicant A is under the age of 16, in addition to the requirements listed in #5, the permission of a County Court Judge is also required. (Judge Cornelius P. Sullivan is the Judge of Juvenile Domestic Relations Court for Burlington County)

Blood Tests are no longer required as of November, 1997.

 

 

Domestic Partnership Certificates

Requirements for obtaining a Domestic Partnership Certificate in the Township of Evesham

Applicants do not need to apply in the town in which they live.

The application fee is $28.00.

Applications are taken by appointment only.

Applicants must execute an Affidavit of Domestic Partnership in the presence of a Notary Public

File the notarized Affidavit of Domestic Partnership form with the Local Registrar of Vital Statistics in any municipality in New Jersey to obtain a Certificate of Domestic Partnership

Applicants must bring with them the following:

  • Current drivers license or photo ID with current address.
  • Provide proof of joint financial responsibility as evidenced by one of the following:
  • Joint deed, mortgage agreement or lease
  • Joint bank account
  • Designation of one of the persons as primary beneficiary on the other person's will:
  • Designation of one of the persons as primary beneficiary in the other person's life insurance policy or retirement plan; or
  • Joint ownership of a motor vehicle
